Mail/FAX Order Form
To place an order by mail, print and complete the following form and mail or FAX it to us with your check, money order or credit card information. Make checks payable to NoOutage.com LLC. Or even better, place your order online without your credit card data, print the checkout screen, write in your credit card info, and mail or fax it to us. Using this online method is better because it tells you the exact shipping & handling amount. The formula below is only approximate. We will confirm your order by e-mail and notify you when it ships. We will also include the shippers tracking number(s) if possible so you can monitor it in transit if desired.
